The group activity The scenario involved a hypothetical patient having a difficult time dealing with a parental figure who was both an alcoholic and a perfectionist. The patient was 19 years old, a white American male, still living home and currently unemployed after being let go from a modest-paying production salary. Part of the job perks had been the ability to use deductions from salary to pay for ongoing education, which was unaffordable due to the mother’s excessive costs of drinking and therefore the individual was currently not attending school. To make matters more difficult, the father consistently pressured the now-adult son to find a better path for himself and generally made the environment unsuitable and unsatisfactory living. Coupled with this, the loss of a trusted girlfriend due to an unexpected car accident just four months prior, the hypothetical psychological patient was withdrawing from others, but still remained motivated to become independent of his parents’ unsatisfactory environment. To identify the culture clash involved, it was necessary to outline the entire case study and all of the situations that came along with it. This youth was typically well-adjusted and the group was to come up with social and psychological strategies, or a blend of them both. The person in the group was to take on the role of a trusted friend who happens to be visiting the house during a period where the father is harassing the son in front of the polite company and obviously belittling his talents and attitudes. To add further fuel to the fire, the mother begins the same tirade and the arguments turn into a heated explosion where the entire group is at each other’s throats. In the group, there were primarily white Americans. However, there was one female Chinese group member, one obviously Middle Eastern individual, and an African-American male. The solution offered by the author involved taking a passive stand and just allowing the argument to continue unless the arguing family members asked for opinion or directed comments in the guest’s direction. This was to ensure neutrality in the case. When the group members gave their opinions and emotional responses to how I had chosen to handle this issue, most of the responses from virtually all cultures were criticizing. They seemed to believe that in order to end the chaotic environment that was leading to what looked like physical hostility, it was my responsibility to interven e and try to calm the situation. Conflict negotiation in this case was deemed best to be disruptive and preventative using appropriate soft language. I wholeheartedly disagreed with the group. I compared three hospitals in my general area to ten different data points that were also compared to the state and national averages. The areas within the statistics I compared were in timely effective manor, survey of patients experience, readmissions, complications deaths, and use of medical imaging. Following the data comparisons, I included a brief explanation of the data point being examined, along with potential causes, improvement suggestions, and outliers when found. Timely effective manor First data point: Outpatients having surgery who received an antibiotic at the right time (within one hour) before surgery. Hospital staff should make sure surgery patients get antibiotics at the right time. It is important to receive antibiotics pre-operation due to the possibility of wound infections from surgical procedures. Research has shown patients who have received antibiotics prior to surgery were less likely to get wound infections. Potential causes for the problem: Each staff member needs to follow a respiratory protocol, and if pneumonia is identified, that protocol needs to be followed. Blood cultures are not performed, and the potential for giving the patient the wrong antibiotic is high. Protocols are not followed, no accountability, or in-service are being regularly held. Process improvement suggestions to improve the results: All three of the hospitals require improvement and need to make sure patients whose initial emergency room blood culture was performed prior to the administration of the first antibiotic in order to provide the most appropriate care. Potential causes for the problem: Health care staffs are not fully aware of the pain each modality can cause and influence the outcome of patient care. They tend to be removed, for example a patient has an abscess to his lower leg, yet he says he is in pain but I am removed from his discomfort because all I see is an abscess. Third data point: Patients who reported that staff always explained about medicines before giving them to them. Patients want to know, they want to be informed and feel about the medications they are taking while in the hospital. It is further important to know what patients are feeling about their hospital experience and by asking patients if their nurse communicated with them about their medication before dispensing it will effectively let the hospital staff know where they need improvement in order to enrich the patients overall hospital experience Results: Florida Hospital Flagler and the state average tied at 60%, Florida Hospital Memorial rated 65% and Flagler Hospital rated 57% and the state at 64%. All comparisons data points fell short of an acceptable 100%. Potential causes for the problem: Protocols not being followed, no in services to remind nurses to communicate with their patients more effectively. Some of the characteristics included are age of the patient, past medical history, and co-morbidities that the patient had when they were admitted to the hospital and a known health risk that can to increase the patient’s risk of dying or of having an unplanned readmission. Process improvement suggestions to improve the results: As performance improvements are identified then appropriate problem-solving techniques can be employed. Beginning with putting together an action plan, that can change certain patterns set by physicians that have been identified as possible contributors with the death rates for heart attack patients. Also, accountability measures should be enforced to hold appropriate department staff accountable for protocol adherence, critical paths to follow through, and enforcing the plan towards improving outcomes, policies that enhance efficiency, and fluid communication across departments. Medical Imaging First data point: Outpatient computed topography (CT) scans of the chest that were combination (double) scans. This data point only applies to Medicare patients that were treated in the hospital and does not include ambulatory or inpatient settings of the hospital. The lower the percentage the better and points to the efficient use of medical imaging for that hospital being compared. The day of owning a home was a wonderful experience that Janet Wilson discovered after closing on her first home. She’s a single mother with two sons and when her loan officer explained that her mortgage loan would be approve and she felt her dream of homeownership will be reality. The closing costs were at a minimum due to the loan officer explained it would be an easy closing with low out of pocket costs. Janet worked for a healthcare organization for several years and was able to save enough money to buy needed furniture for her home. Janet enjoyed her new home and finally felt the independence and accomplishment in her life. At 45 years old, she waited a long time witnessing others in her family and friends reaching that goal of homeownership. Janet desperately wanted to reach that goal as well as experience the feeling of obtaining the American dream. In three years of homeownership she was able to pay her mortgage with no problem and was able to start to re-decorate her home. Her boys loved the neighborhood and had developed close friends their age. Ms.  Wilson forgot all her about her closing until one day she received in the mail a letter from her mortgage company that her interest rate is being increased. Janet saw her new adjusted mortgage payment and she almost fainted with disbelief.
